Work Permits

If you are a current Columbus City Schools student, between 14 and 17 years old, and want to work during the school year, you must complete a Work Permit Application. There are three sections that must be completed:

• Students' Information
• Employer Information
• Physicians Approval 

*It is important to note that the student must be present to process the work permit application.

Download and print the Work Permit application below. The completed form must be presented to the Division of School Choice, located in the Central Enrollment Center at 430 Cleveland Avenue between 8:30 a.m. and 4:15 p.m. Monday - Friday.
